Audit readiness: build a traceable evidence chain

Trace a real event
A common preparation problem is having procedures without being able to explain actual practice. Select a recent delivery, defect or equipment exception and trace the requirements, actions, approvals and verification. Breaks in that trail reveal interfaces that need attention.
Clarify the audit purpose and criteria. Internal, customer or purchasing-party audits and independent certification audits can have different objectives.
Connect four kinds of evidence
| Stage | Question | Possible evidence |
|---|---|---|
| Requirements | What was agreed, and by whom? | Contract, drawing, process standard |
| Execution | Who performed the work and under what conditions? | Work order, operation and handover records |
| Results | How was conformity evaluated? | Inspection or acceptance records |
| Improvement | Was a deviation effectively addressed? | Cause tests, actions and follow-up |
These are examples, not a mandatory document set. Evidence should be authentic, attributable and retrievable.
Examine weak interfaces
Sample revisions, outsourced activities, supplier exceptions, temporary role cover and complaints. Correct the immediate control problem, then verify its cause and the effectiveness of the revised routine. Replacing an obsolete drawing is only part of fixing a drawing-control failure.
Ask people to explain their normal work rather than memorize answers. Use preparation reviews to find weaknesses in the system.
